Shoulder Impingement: Symptoms, Causes and Treatment Options

Is Shoulder Impingement the Same as a Rotator Cuff Tear?
No.
They are related but different.

Rotator cuff tear
Part or all of one of the tendons may be structurally torn.
Some patients with a rotator cuff tear still have symptoms described as “impingement”, which is why clinical examination and imaging may sometimes be useful when the diagnosis remains uncertain.
Is Shoulder Impingement the Same as Frozen Shoulder?
No.
This distinction matters.
Shoulder impingement
Movement may be painful, but the shoulder usually still has considerably more passive movement.
Frozen shoulder
The shoulder becomes genuinely stiff.
Both:
- Active movement
- Passive movement
become restricted.
If someone else cannot move your shoulder much farther than you can move it yourself, frozen shoulder becomes more important to consider.
What Is the Difference Between Shoulder Impingement and Arthritis?
Arthritis affects the joint surfaces.
It may cause:
- Joint stiffness
- Reduced movement
- Grinding
- Pain with general shoulder use
Shoulder impingement is more closely associated with the rotator cuff and surrounding soft tissues.
X-ray may be more useful when arthritis is suspected, while ultrasound or MRI may be more useful for rotator cuff assessment.
How Is Shoulder Impingement Diagnosed?
A medical assessment may include checking:
- Where the pain is located
- Which movements reproduce it
- Shoulder strength
- Active movement
- Passive movement
- Rotator cuff function
- Whether the neck could be contributing
The examination should also consider alternative diagnoses such as:
- Frozen shoulder
- Rotator cuff tear
- Arthritis

Do I Need an X-Ray?
Not necessarily.
X-rays can be useful when the doctor wants to assess:
- Arthritis
- Bone abnormalities
- Previous injury
- Joint alignment
But X-rays do not directly show rotator cuff tendons well.
Is Ultrasound Useful?
Yes.
Musculoskeletal ultrasound can assess:

Should I Stop Using the Shoulder?
Usually not completely.
The aim is generally to reduce activities that strongly aggravate symptoms while keeping the shoulder moving within a tolerable range.

- High-volume racket sport
while continuing:

Can Physiotherapy Help?

What Exercises Are Usually Used?
Exercises depend on the individual.

- Gradual resistance training
There is no single exercise that works for everyone.
Should Exercise Hurt?
A small amount of discomfort can sometimes occur during rehabilitation.

Progression should be gradual.
What if Physiotherapy Is Not Helping?

- Is frozen shoulder developing?
- Is arthritis contributing?
- Is the neck involved?
- Would imaging change treatment?
Repeating the same programme indefinitely may not be useful when progress has clearly stopped.
Can a Steroid Injection Help?
A corticosteroid injection may provide temporary pain relief for selected patients, particularly when pain:
- Interferes with sleep
- Prevents normal movement
- Makes physiotherapy difficult
Mayo Clinic notes that injections may help temporarily, although repeated steroid exposure can weaken tendon tissue and may affect future surgical outcomes.
For this reason, injections are not automatically appropriate for every patient.
Does an Injection Fix the Problem?
No.
A steroid injection may reduce:
- Pain
- Inflammation
but it does not necessarily correct the underlying tendon problem.
Its main role is often symptom control.
What About Shockwave Therapy?
Shockwave therapy is used more commonly for selected chronic tendon conditions, particularly when tendon degeneration or calcific deposits are present.
It should not be assumed to be necessary for every patient with shoulder impingement.
The diagnosis should determine whether it is appropriate.
What About Massage or Manual Therapy?
Manual therapy may provide short-term symptom relief in some patients.
However, longer-term recovery usually depends more on:
- Restoring movement

Passive treatment alone may not be sufficient if weakness and load tolerance remain unaddressed.
Can Shoulder Impingement Heal Without Surgery?
Yes.
Many people improve with non-surgical treatment.

When Is Surgery Considered?
Surgery is generally not the first treatment.
It may be discussed when:
- Symptoms remain substantial despite appropriate rehabilitation

The decision depends on the diagnosis rather than the label “impingement” alone.
Does an MRI Abnormality Mean I Need Surgery?
No.

- Range of movement
- Functional limitations
- Response to treatment
An abnormal scan alone does not determine treatment.
What if My Shoulder Hurts Mainly at Night?
Night pain can occur with:

If night pain repeatedly wakes you and is not improving, reassessment may be reasonable.
What if I Cannot Lift My Arm?
This deserves closer assessment, particularly if it followed:

Earlier imaging may be appropriate in this situation.
Can Neck Problems Mimic Shoulder Impingement?

- Weakness
If symptoms travel below the shoulder into the arm or hand, the neck should also be considered.
